This handout is given to my students in grades 4-6 prior to leaving for their Christmas holiday. It is a checklist of activities and challenges for them to complete during their holiday. It encourages them to stay active each day, as well as maintain a healthy diet.

Teaching Suggestions:

I use this worksheet to provide feedback on the students likes and dislikes with types of activities. It also gives me a chance to see what obtacles they face at home when it comes to exercise and nutrition.

The purpose of this activity is not for grading, its goal is to increase student participation in physical activity at home.

To assist with motivation, I also have a drawing for prizes for those worksheets turned in. My prizes are small items left over from JRFH, etc.
